TL;DR

Cass, Saunders and Washington counties each have more than a fifth of residents 65 or older but only a handful of licensed ALFs and nursing homes. Plattsmouth, Wahoo, Ashland and Blair hold most of them. Saunders County's Area Agency on Aging is Aging Partners, not ENOA.

Older counties, shorter lists

In Census Bureau estimates for 2025, every one of the three outer counties had a higher share of residents 65 and older than Douglas or Sarpy. Their licensed senior care is concentrated in the county seats and a few larger towns, so a parent in Weeping Water, Yutan or Arlington may have no licensed building in town.

Cass County: Plattsmouth and Louisville

Cass County's licensed buildings are in Plattsmouth and Louisville. The Nebraska Masonic Home in Plattsmouth is a long-standing campus with both assisted living and a state-licensed nursing facility. Cass County has no hospital on the DHHS hospital roster, so hospital care means Bellevue, Papillion, Omaha or Lincoln. ENOA serves Cass County, including Meals on Wheels.

Saunders County: Wahoo and Ashland

Saunders Medical Center in Wahoo is the county's critical access hospital and also runs a nursing facility. Wahoo and Ashland have the county's licensed assisted living, including a large Ashland community with memory care. Saunders is outside ENOA's area: its Area Agency on Aging is Aging Partners in Lincoln, (402) 441-7070 or (800) 247-0938, which matters when families call for Meals on Wheels or care management.

Washington County: Blair and Fort Calhoun

Washington County has the highest share of older residents of the five counties. Memorial Community Hospital in Blair is a critical access hospital, and Blair's licensed senior care includes long-standing nonprofit homes. Fort Calhoun has one licensed ALF. ENOA serves the county, and Immanuel Pathways' PACE program covers select Washington County ZIP codes.

Distances that shape decisions

Straight-line distances from the county seats to downtown Omaha run from roughly 20 miles from Blair and Plattsmouth to more than 25 from Wahoo, and drive times are longer. For a spouse who does not drive at night, that distance can decide whether a move to an Omaha suburb is realistic. Our county pages show the distance from each county seat to the nearest hospitals and to Omaha, so families can weigh local options against the larger rosters in Elkhorn, Papillion and Bellevue.

Making the most of a short list

In a county with two or three licensed buildings, the first visit matters more. Ask each one for its latest DHHS survey, its rate schedule and its discharge criteria, and ask what happens if your parent's needs grow: is there a nursing facility on the same campus, as at the Nebraska Masonic Home in Plattsmouth and Crowell Memorial Home in Blair, or would a second move be needed? Ask whether the building is waiver-certified, since a resident who spends down in a private-pay-only building would usually have to move, and in a rural county the next certified option may be in another town.

Hospitals and nursing homes in the outer counties

Two of the three counties have a critical access hospital: Memorial Community Hospital in Blair and Saunders Medical Center in Wahoo. Critical access hospitals are small by design and transfer patients who need specialized care to larger hospitals, often in Omaha or Lincoln. Each outer county also has certified nursing homes, and our facility pages show their CMS ratings, staffing and latest inspections. For a parent recovering from surgery, a nursing home close to the family may matter more than one close to the surgeon, but ask how the home coordinates follow-up visits in the city.

When the local list is too short

Families in the outer counties usually compare local buildings with options in Elkhorn, Papillion, Bellevue or Omaha, weighing distance against fit. Before deciding, check the local buildings' waiver and memory care flags, because a short list leaves little room to move later. Staying home with help is often more realistic here than in the city: ENOA's in-home services, the AD Waiver and adult day programs in nearby towns can bridge a long time. Ask about each service's coverage area during the first call.

Whatever the choice, ask every provider about coverage area at the first call. Home care agencies, hospices and PACE all serve some rural ZIP codes and not others, and Meals on Wheels routes depend on the county's Area Agency on Aging, which is ENOA for Cass and Washington and Aging Partners for Saunders. A ten-minute call can save a week of planning around a service that does not reach your parent's road.
