What is in the Omaha-area senior care directory?

The directory lists every assisted-living facility and nursing home on Nebraska's DHHS rosters for Douglas, Sarpy, Cass, Saunders and Washington counties, plus licensed adult day services and CMS-certified home health agencies and hospices, 167 in all, with CMS Care Compare ratings for the 36 certified nursing homes.

Where does the directory's data come from?

Only from official bulk data: the DHHS Licensure Unit's assisted living, long-term care and adult day rosters (updated October 15, 2026) and CMS's Provider Information, Home Health and Hospice datasets; we do not use aggregator listings, operator marketing or reviews.

Every listing page carries a “Source” line with the data date. Facility pages exist for every licensed ALF and every CMS-certified nursing home; home health agencies, hospices and adult day services appear as roster rows.

How do I use the listing pages?

Each listing page has a search box and filters for county, city and type (and CMS rating for nursing homes), and every column can be sorted by clicking its heading, so you can, for example, list every waiver-certified ALF in Sarpy County.

Why are there no inspection dates for assisted living?

Because Nebraska does not publish assisted living inspection results in bulk: DHHS makes them viewable one facility at a time through its LIS license search, so the directory shows what the roster carries and points you to the lookup.

Nursing homes are different: CMS publishes inspection dates and deficiency counts for every certified home, and those appear in the nursing home listing and on each facility page. For an ALF, ask for the latest DHHS survey; residents have a right to examine it under 175 NAC 4-006.04.
